As part of the terms of employment, employees in this role are required to join the union within 31 days of hire and remain a member (e.g. dues paying, fee paying, religious exception contributor) for the duration of the collective bargaining agreement. **What You Will Do** Collaborate with the interdisciplinary teams in the delivery of quality patient care under the direction of a registered nurse, assists in performing personal care needs and compassionate end-of-life care. Contribute to overall unit functions. **Required Qualifications** + California Home Health Aide (HHA) - CA Department of Public Health -REQUIRED + California Certified Nurse Assistant (CNA) - CA Department of Public Health -REQUIRED + AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED + Driver's License - CA Department of Motor Vehicles -REQUIRED + ServSafe Food Handler Certified - National Restaurant Association -REQUIRED **Preferred Qualifications** + 1 Year experience in acute care, home health, or hospice. **Other Qualification Requirements** + Current Food Handler card or acquisition within 90 days of hire.Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines. **Essential Functions** + Plan of careObserves patient condition and reports changes to the RN.Reports pertinent patient related information to RN.Observes changes in patient's condition such as declining ability to participate in ADL's, changes in skin condition, level of consciousness, pain management, etc.Promptly reports appropriate changes in patient's status to RN.Effectively communicates and participates in the coordination of the plan of care to meet established goals. + Departmental compliancePerforms routine equipment maintenance according to departmental policy.Maintains all certifications required of the job.Demonstrates and maintains expertise in through attendance and participation in department in-services and meetings.Current CPR CertificateBody MechanicsDisaster ProceduresFireHazardous MaterialsInfection ControlAttendance at unit specific mandatory in-services.Mandatory/non- mandatory staff meetings: reviews staff meeting minutes.Attendance at additional in-services per unit standard and expectation.Monitors and maintains CEU record per department policy and expectation. + Effective communicationUnderstands and demonstrates effective communication to patients/families in alignment with hospice philosophy and patients plan of care.Provides appropriate discipline specific information on end of life issues to patient/families based on their needs.Listens actively and provides a meaningful presence to facilitate the identification and discussion of patient goals and preferences as it relates to personal care.Promotes rapport to foster patient relationship to support patient throughout their hospice care. + Patient careProvides comprehensive personal care to patients under the supervision of the RN.
